Complete this form to register your organisation as an NDIS implementing service provider in the NSW RPA System.

Before completing the form, please note:

• Your organisation must be registered with the NDIS Quality and Safeguards Commission (NDIS Commission)
• All details included must match those listed per the NDIS Provider Register
• Your organisation must nominate two staff as users (including first name and surname, and email address in the user’s name):
  1. 'Service Provider Administrator' who can manage your organisation’s profile, create new users, create and lodge submissions, create participants, and convene panels
  2. 'Service Provider User' who can create and lodge submissions, create participants, and convene panels

Failure to comply with the above conditions may result in your application being rejected.

• Organisation’s registration with the NDIS Commission has expired
• Organisation is presently in an audit phase of its registration
• Organisation does not implement restrictive practices (e.g., it only develops behavior support plans)

If you are not registered with the NDIS Commission or are awaiting the outcome of your registration request, you are not eligible for registration on the NSW RPA System. Please apply once you are registered with the NDIS Commission.
